What changes for you

ccs coordinator --adopt now fails up front rather than surprising you later.

You adopt Before Now
A session named my hub Accepted, then ccs coordinator could never reopen it Refused, saying to rename it first (/rename, or Ctrl+X E)
A closed session by id Came back renamed coordinator Keeps its name
A name two running sessions share Picked one at random Refused, asking for the id
A session when you already have a coordinator Silent Says the old one stays as an ordinary session

A session already adopted under a name with a space still can't be reopened by ccs coordinator, and the error says why. Enter on its row still resumes it.

Proof

Reran the reproduction with the built binary, an empty HOME and a fake claude. The space-in-name adoption is now refused with nothing recorded. The rest are covered by new tests.

A name with a space adopted fine and then couldn't be reopened, a closed
session adopted by id came back renamed coordinator, and two running sessions
with one name picked one at random. It now refuses those it can't reopen and
ambiguous names, keeps a session's name, and says when it replaces another.
